Obstetric physiotherapy consists of rehabilitation of women with gynecological issues. The main focus is on women’s health and improving conditions related to the pelvic floor muscles. The pelvic floor muscles support the womb, the bladder and the bowels.

 

Many factors can weaken the pelvic floor in women, such as pregnancy, childbirth, aging, and weight gain. Pelvic girdle pain may radiate down the thigh, with or without pain in the pubic symphysis.

Also included in this is breast cancer (conservative)and rehabilitation post-surgery (Modified Radical Mastectomy). Breast cancer treatment may lead to chronic pain, decrease in muscle strength and reduced cardio respiratory fitness. This is due to the surgical trauma and scarring, along with the fibrosing effect of adjuvant radiation therapy. Impaired shoulder function and lymphoedema is frequently seen after the treatment of breast cancer and early rehabilitation and exercises improve shoulder mobility, decreases upper limb dysfunction and reduces chest pain. Physiotherapy intervention includes patient education, counseling and encouragement, chest physiotherapy, ROM exercise and edema prophylaxis.

This type of rehabilitation can also include antenatal and postnatal therapeutic exercises.

 

Pregnancy-related low back pain and pelvic pain are common musculoskeletal pain which can affect daily activities such as walking, work, sleep, mood, thereby consequently reducing the quality of life. More than 50% of women experience pain in the lumbopelvic area during pregnancy. Physiotherapists use several treatments for pregnancy-related lumbopelvic pain, including passive treatments such as mobilization and active treatments such as exercises. Education regarding posture, back care, and modification of daily activities all help to ensure optimal postural alignment, which thereby helps minimizes joint stress in pregnant women. This can be achieved via physical therapeutic exercises, aerobic training, muscle strengthening, breathing, stretching and flexibility which provide muscle strength, positive psychological effects and improvement in body posture.

Diastasis recti and some abdominal separation during and following pregnancy is expected. During pregnancy, the abdominal muscles and connective tissues are stretched out from the expanding uterus and pushing during delivery can also lead to diastasis recti.  These structural changes can limit abdominal muscle function, decreasing the ability of the abdominals to provide stability to the pelvis. Cosmetically this appears as bulging of the abdominal wall, and it may also alter the body image of the individual. A PHYSIOTHERAPIST WILL PLAN AN INDIVIDUALIZED EXERCISE PROGRAMME TO HELP IMPROVE MUSCLE STRENGTH AND REDUCE PAIN. This includes core muscle activation, correct body mechanics and posture awareness, along with pain relief interventions.

 

Urinary incontinence is the involuntary leakage of urine. Urinary incontinence is a common problem that affects many people. Control over the urinary sphincter is either lost or weakened, which causes the person to urinate when they do not want to.  One commonly known type of incontinence is stress incontinence. Urine leaks when you exert pressure on your bladder by coughing, sneezing, laughing, exercising or lifting something heavy.

 

Kegel exercises or Pelvic floor exercises can be very effective in treating stress incontinence and make the muscles of your pelvic floor stronger. This is a specialized form of therapy to help with pelvic floor muscle coordination and bladder habits, and to improve/resolve symptoms of urinary urgency, frequency, and leakage. Biofeedback training and electrical stimulation therapy are also techniques used by physiotherapists to help women identify, isolate and contract the pelvic floor muscles
